What services does Harmony Horizon provide?
We provide Supported Community Living (SCL) and Home-Based Habilitation to individuals across Iowa on the ID and BI waivers, and to habilitation clients.
Which waivers do you serve?
We serve the Intellectual Disability (ID) and Brain Injury (BI) Medicaid HCBS waivers, and habilitative clients outside waiver funding.
How do I get started or refer someone?
Individuals and families can contact our team, and case managers or referral sources can complete our online referral form. We'll follow up to schedule an initial meeting.
How are services paid for?
Most services are funded through ID or BI waiver funding, habilitation service billing, or private pay.
