Harmony Horizon Services was founded on a simple belief: that every individual deserves support built around their own goals, in the community of their choosing.

Our Story

Rooted in the Iowa community

Individuals and staff engaged together in a Harmony Horizon program activity

Harmony Horizon Services was created to bring genuinely person-centered home and community-based support to individuals across Iowa. We saw too many people navigating a system that treated services as a checklist, and we set out to do it differently.

Today we provide Supported Community Living and Home-Based Habilitation to individuals on the ID and BI waivers and to habilitation clients, always starting from the same place: the person, their goals, and their definition of a good life.

Luah Soko

Co-Founder

Luah Soko is a co-founder of Harmony Horizon Services LLC with over ten years of direct care experience as a Direct Support Professional (DSP). She has worked with individuals with intellectual disabilities, traumatic brain injuries, and those receiving habilitative services, building close, trusting relationships along the way.

Luah's approach is rooted in meeting people where they are, as she believes it is important for the individuals she serves to feel and be in control of their own lives. People can live their best life at any stage of their lives.

She holds a Bachelor of Science in Psychology and is currently pursuing a Master of Social Work. Luah's vision for Harmony Horizon Services is an environment where individuals live meaningful, fulfilling lives supported by dedicated staff, with person-centered care at the core of everything she does.

Reuben Azoza

Co-Founder

Reuben Azoza is a co-founder of Harmony Horizon Services LLC and brings more than a decade of hands-on caregiving experience, along with professional service as a Brain Injury Specialist across four healthcare organizations. His interest in the field was shaped early on while supporting the care of his younger sister during her experience with sickle cell disease. That experience provided him with a clear understanding of the challenges families face during serious health conditions and guided his long-term commitment to supporting individuals with disabilities in achieving greater independence and quality of life.

Reuben holds an Associate of Science degree and a bachelor's degree in Multidisciplinary Science from the University of Iowa. He also maintains strong ties to the local community, having attended Jefferson High School prior to his studies at the University of Iowa. Through Harmony Horizon Services, Reuben is dedicated to building a lasting presence in Iowa by delivering reliable, person-centered support and developing services that improve outcomes for the individuals and families he serves.
