Refer an individual for Supported Community Living or Home-Based Habilitation services. Our team will follow up to schedule an initial meeting.
How to refer
Three steps to submit a referral
1. Download the referral form
Download the HCBS Member Referral Form (PDF) and complete it as fully as you can. Fields marked with an asterisk are required; if a section doesn't apply, enter N/A.
2. Return it securely
Send the completed form to Harmonyhorizonservices@gmail.com using a secure/encrypted email method. This form may contain protected health information, so please only submit it through a secure channel.
3. We follow up
Our team reviews the referral and follows up to schedule an initial virtual meeting. If we can meet the applicant's needs, we'll arrange an in-person intake. (We're happy to accommodate applicants who can't meet virtually.)

The complete referral form covering member information, service requested, supports, medical and behavioral needs, and requested documents. Complete it and return it to Harmonyhorizonservices@gmail.com by secure email.
