Waiver & eligibility information
Harmony Horizon Services supports individuals across Iowa through Medicaid HCBS waivers, as well as habilitative service clients outside waiver funding.
Intellectual Disability (ID) Waiver
Serves individuals with an intellectual disability who need support to live in the community rather than an institutional setting. Harmony Horizon provides Supported Community Living and Home-Based Habilitation to ID waiver participants.
Brain Injury (BI) Waiver
Serves individuals with a qualifying brain injury who need support to remain in or return to community living. Harmony Horizon provides Supported Community Living and Home-Based Habilitation to BI waiver participants.
Habilitation Services (Non-Waiver)
We also serve individuals accessing habilitation services outside of ID or BI waiver categories, helping ensure more members of our community can access person-centered support.
